Seeking a Case Manager in the Adult Family Care (AFC) Program who will work independently and collaboratively within the AFC team to ensure appropriate services for assigned participants in AFC program.
The AFC Case Manager will be a primary agency contact for AFC caregivers. The Case Manager will work collaboratively with the AFC Caregiver, the AFC Participant and the AFC RN to encourage, support and address the medical, social and psychological needs of the AFC participants. The AFC Case Manager plays an important role in connecting caregivers and participants with the tools they need to further/develop skill building and to facilitating linkages with other community based and natural supports, as well as navigating state and local resources.
Candidate must be available to work a flexible schedule which may occasionally include early morning or early evening hours as home visits may be scheduled to meet the needs of the AFC participants and caregivers.
