Spurwink's Lewiston office is looking for a care coordinator/case manager to join our Children's Behavioral Health Home team! In this model, you will participate in a team-based approach to care which also includes Clinical Team Leader, Family/Youth Support Partner, Nurse Care Manager, and Psychiatric and Medical Consultants.
Care coordinators work collaboratively with clients/families to address mental health, behavioral and medical needs of the client. You will work closely with all other providers, attend school meetings or other appointments. Our program works closely with many Lewiston and neighboring schools. We also work closely with other Spurwink programs such as day and residential services, as well as working with clients participating in Spurwink's Failure to Thrive and Child Abuse Programs.
You will work with a diverse population of families within this immediate Lewiston community, and beyond. Our office covers 4 counties: Androscoggin, Sagadahoc, Franklin and Oxford
As a Care Coordinator, you will assess needs with the family during intake, develop treatment plans, monitor services and are responsible for collaboration between providers. You will connect families with needed resources, and help address and work through any barriers. This work is mostly home and community based, although families are welcome to come meet in our office in downtown Lewiston as well.
Care coordinators need to have strong time management skills to manage family and documentation needs. Care coordinators need to be tech savvy as you will work within various computer programs. You will need to be able to juggle tasks, prioritize needs, meet family needs, as well as complete required documentation within each week.
Bachelor’s degree from an accredited four (4) year institution of higher learning with a specialization in social services or closely related field. Experience in social service preferred.
